There are not as many that involve jail, but there are a variety that involve ruined lives and even bankruptcy. I personally think it was the corporate governance that failed, not the software development and debugging process. The legal system was complicit and enlarged the overall consequences, but the but for test tells me that it was the poor corporate governance that was at fault for a root cause.
