I'm currently fighting this exact fight with Blue Shield I was getting nowhere until I started connecting with and then publicly shaming their executive leadership on LinkedIn. If you do that, you get transfered to their "executive relations team" who still are unable to get things done, but it's at least a different department you can file grievances that go nowhere with.
